Mr. Jerimiah Louis Matt, 17, of Madisonville, KY died at 5:15 P.M. Wednesday, November 3, 2004 at the Vanderbilt Medical Center in Nashville, TN. \ He was born October 23, 1987 in Poplar Bluff, Missouri. \ He is survived by his parents, Michael and Terry Kirkman Matt of Madisonville; one sister, Tiffany Lynn Matt of Madisonville; one brother, Jordan Michael Matt of Madisonville; paternal grandparents, Robert L. and Linda Matt Sr. of Dudley, MO; maternal grandmother, Lena Kline of Poplar Bluff, MO; great-grandparents, Lucille Radford of Dudley, MO and Pearl Matt of Cahokia, IL; several aunts uncles and cousins. \ Funeral services will be 3:00 P.M. Saturday, November 6, 2004, at Concord General Baptist Church in Manitou, KY with Rev. Eddie Fleming. \ Visitation will be at the Church after 1:00 P.M.
